Do you have a current Will or Trust? Do your checking and savings accounts have only one signature on file? When was the last time you updated your beneficiaries on securities, bank accounts or life insurance? Do you have a Safety Deposit box and who has access to it? Can you put your hand on your original birth certificate issued by the state you were born in?    Marriage license? Title or Deed to all properties? Do you know the city and state that your parents and your spouse’s parents were born in? Have you discussed and written instructions in case of death?   Who has a copy of these wishes? Do you have immediate access to the Social Security Numbers for everyone in your family? Do you have a Medical Directive in case of emergency? Who has a copy of it? Do you have a current list of medications that you take (including over-the-counter) as well as  for all family members? Do you have a copy of your credit cards? Where are you keeping necessary financial records? (i.e. taxes, bank statements, income records) Do you have a printed list of the phone numbers stored in your cell phone?
